Four individual meetings to review care plans, address any concerns with healthcare provider, develop health goals and monitor progress
Clients will meet with a Health Adviser to take and review information provided on your personal health appraisal
Discuss relevant medical history and personal goals with the client
Provide tips on how the client can confront their health challenges to reach their goals
Client will be introduced to what medical advocacy is and how to ensure they are getting the care they need from medical providers, facilities and practitioners
